PLEASE NOTE:  Vendor will receive links to download their digital venodr registration packet in the thank you page of the checkout, along with an emailed link that will last for 30 days.  PLEASE check in those two places for the forms.

By submitting payment and returning your vendor information form, you agree to have read and understand Mockingbird Farm Sanctuary’s Fall at the Farm Festival and Fundraiser’s rules, regulations and contract for 2024.

After payment has been completed, please download the .zip file to access all forms.

Please fill out your vendor information form and return to mockingbirdfarmny@gmail.com by October 1st, 2024.

If required, health permits must be submitted through Genesee County NY Health Department no later than September 27th, 2024.

To-do list:
-Vendor payment
-Liability insurance (naming Mockingbird Farm Inc. and Jonell Chudyk as certificate holders) 
-Submit (mail, e-mail, in person) the vendor information form
-Secure health permit through Genesee County if needed
